How it works

The mechanism of action of voriconazole is inhibition of cytochrome P450 (CYP 450)–dependent 14a-lanosterol demethylation, which is a vital step in cell membrane ergosterol synthesis by fungi. For yeasts, voriconazole appears to be fungistatic, as are other azoles.

Side Effects

The most common side effects are nausea, altered bowel habit (constipation, diarrhoea, or flatulence) abdominal pain, and visual disturbances. If you get visual disturbances these usually get better over a few days or weeks and do not damage your eyes. Some patients experience troubling dreams, poor concentration or unsteadiness standing or walking. Voriconazole can cause peripheral neuropathy.

Drug Interactions

Medications can affect the removal of voriconazole from your body, which may affect how voriconazole works. Examples include butalbital, efavirenz, mitotane, rifamycins (such as rifabutin, rifampin), ritonavir, secobarbital, certain drugs used to treat seizures (such as carbamazepine, phenobarbital).

Indication

Treatment of invasive aspergillosis, Treatment of candidaemia in non-neutropenic patients, Treatment of fluconazole-resistant serious invasive Candida infections (including C. krusei)., Treatment of serious fungal infections caused by Scedosporium spp. and Fusarium.

When not to Use

Hypersensitivity to any ingredient of the preparation. Hereditary fructose intolerance, hepatic coma, azotemia, congestive cardiac failure, severe acidosis and disturbances of amino acid metabolism.

Precaution

Caution should be used in prescribing Voriconazole according to patients with hypersensitivity to other azoles.

Warnings

Warning 1

Do not take a double dose to make up for a forgotten dose.

Warning 2

Your liver and renal function will be monitored either in clinic or at your General Physician surgery whilst you are on antifungal therapy.

Warning 3

The tablets and liquid should be taken on an empty stomach, either an hour before or 2 hours after food.
