How it works

Atropine acts as a competitive antagonist of the parasympathetic (and sympathetic) acetylcholine muscarinic receptors. Topical atropine on the eye induces mydriasis by inhibiting contraction of the circular pupillary sphincter muscle normally stimulated by acetylcholine. This inhibition allows the countering radial pupillary dilator muscle to contract which results in dilation of the pupil. Additionally, atropine induces cycloplegia by paralysis of the ciliary muscle which controls accommodation while viewing objects.

Side Effects

• Increased sensitivity to light (photophobia). Eyes should be protected from bright light, particularly when pupils are widened (dilated). • Dry mouth • Difficulty in swallowing and talking • Flushing and a dry skin • Slower heart beat (bradycardia) followed by faster heart beat (tachycardia) • Feeling your heart beat (palpitations) • Irregular heart beat (arrhythmia) • Reduced lung fluids • Urgent desire to pass urine • Inability to pass urine • Constipation • Confusion (particularly in the elderly) • Feeling sick (nausea)

Drug Interactions

Monoamine oxidase inhibitors (MAOI) , Phenothiazines , amantadine , TCADs , antihistamines , domperidone , metoclopramide , disopyramide , mexiletine , ketoconazole , sublingual preps ( e.g , nitrates-due to dry mouth , parasympathomimetics , propofol.

Indication

Atropine is indicated for: Mydriasis , Cycloplegia , Penalization of the healthy eye in the treatment of amblyopia

When not to Use

Atropine sulfate ophthalmic solution should not be used in anyone who has demonstrated a previous hypersensitivity or known allergic reaction to any ingredient of the formulation because it may recur .

Precautions

Precaution

It should only be used with caution in patients who may have raised intra ocular pressure.

Warnings

Warning 1

The product contains benzalkonium chloride solution and soft contact lenses must not be worn during the period of use.

Warning 2

Patients should wash hands after using the eye drops and great care should be taken to avoid getting the product into the mouth.

Warning 3

Darkly pigmented iris is more resistant to pupillary dilation and caution should be exercised to avoid overdosage.
