How it works

Artesunate is metabolized to the active DHA. the endoperoxide bridge of DHA reacts with heme, generating free radicals which inhibit protein and nucleic acid synthesis of the Plasmodium parasites during all erythrocytic stages.

Side Effects

Drug induced fever , Cardiotoxicity , reduction in reticulocyte counts , dizziness, itching, vomiting, abdominal pain, flatulence, headache, body ache, diarrhoea, tinnitus and increased hair loss, macular rash, reduction in neutrophil counts and convulsions , skin rash and pruritus .

Drug Interactions

Antioxidant drugs e.g. Vitamin E,C . Additive effect seen with chloroquine. Antagonistic effect seen with pyrimethamine and sulphonamides.

Indication

Treatment of severe falciparum malaria in areas where there is evidence of quinine resistance.

When not to Use

The drug is contraindicated in patients with prior hypersensitivity to artesunate or artemisinin derivatives.

Precautions

Precaution

Parenteral artesunate should be used for the treatment of severe falciparum malaria only where there is evidence that the antimalarial efficacy of quinine is declining.

Warnings

Warning 1

Monitor patients for 4 weeks after artesunate treatment with Artesunate for Injection for evidence of hemolytic anemia. Since a subset of patients with delayed hemolysis after artesunate therapy have evidence of immune-mediated hemolysis, consider performing a direct antiglobulin test to determine if therapy, e.g. corticosteroids, are necessary.

Warning 2

Hypersensitivity to artesunate including cases of anaphylaxis have been reported during the use of parenteral artesunate (including Artesunate for Injection) . If hypotension, dyspnea, urticaria, or generalized rash occurs during administration of Artesunate for Injection, consider discontinuing Artesunate for Injection administration and continuing therapy with another antimalarial drug.

Warning 3

Artesunate should not be used during the first trimester of pregnancy.
