How it works

Telmisartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin by selectively blocking the binding of angiotensin to the AT1, receptor in many tissues, such as vascular smooth muscle and the adrenal gland. Its action is therefore independent of the pathways for angiotensin synthesis. Amlodipine: Amlodipine is a dihydropyridine calcium channel blocker that inhibits the transmembrane influx of calcium ions by binding to both dihydropyridine and nondihydropyridine sites into vascular smooth muscle and cardiac muscle.

Side Effects

Telmisartan : Impotence, increased sweating, flushing; Body as a Whole: allergy, fever, leg pain, malaise , Palpitation, dependent edema, angina pectoris, tachycardia, leg edema, abnormal. ECG , insomnia, somnolence, migraine, vertigo, paresthesia, involuntary muscle contractions, hypoesthesia , flatulence, constipation, gastritis, vomiting, dry mouth, hemorrhoids, gastroenteritis, enteritis, gastroesophageal reflux, toothache, nonspecific gastrointestinal disorders , gout, hypercholesterolemia, diabetes mellitus . Amlodipine : Arrhythmia (including ventricular tachycardia and atrial fibrillation), bradycardia, chest pain, hypotension, peripheral ischemia, syncope, tachycardia, postural dizziness, postural hypotension, vasculitis , hypoesthesia, neuropathy peripheral, paresthesia, tremor, vertigo; Gastrointestinal: anorexia, constipation, dyspepsia, dysphagia, diarrhea, flatulence, pancreatitis, vomiting, gingival hyperplasia, change of bowel habit.

Drug Interactions

Baclofen, amifostine, neuroleptics or antidepressants , Corticosteroids , Potassium sparing diuretics or Potassium supplements , Lithium , acetyl salicylic acid , CYP3A4 inhibitors like erythromycin , diltiazem , ketoconazole, itraconazole, ritonavir , rifampicin, Hypericum perforatum .

Indication

It is indicated for the treatment of hypertension, alone or with other antihypertensive agents.

When not to Use

The combination of amlodipine and telmisaratn is contraindicated in patients who are hypersensitive to the active substances, to dihydropridine derivatives, or to any of the excipients.

Precautions

Precaution

Periodic monitoring of potassium and creatinine serum levels is recommended in patients with a recent kidney transplant as amlodipine and telmisartan are not dialyzable.

Warnings

Warning 1

In patients with an activated renin-angiotensin system such as volume or salt-depleted patients (e.g; patients receiving high doses of diuretics) symptomatic hypotension may occur after initiation of therapy with combination of amlodipine and telmisartan tablets.

Warning 2

Close monitoring is required when administering amlodipine in patients with severe aortic stenosis as it may cause acute hypotension.

Warning 3

Particular caution should be exercised when administering the combination of amlodipine and telmisartan to patients with hepatic insufficiency or biliary obstructive disorders.
